Membership is FREE!  To apply as a Member of OPHA please fill out the following form and click the SUBMIT button.   Membership to OPHA is, for the most part, automatic as by clicking submit or mailing in your application you agree to all of the terms and alliances that OPHA stands for.  All those agreeing to the OPHA Alliance are welcome.  It is only by breaking the agreement you have signed (digital signature or actual  one as both are binding) that you can be removed from OPHA.
